How Should Hair Transplant Before-and-After Photographs Be Evaluated?

Scientific summary: Different lighting, angles, hair lengths and combing can make outcome photographs misleading. Scientific evaluation requires standardised imaging, trichoscopy when appropriate and disclosure of the actual follow-up interval.

Why standardise before-and-after photographs?

Appearance depends on lighting direction, camera distance, head tilt, wetness and combing. Changing these can make the same patient appear more or less dense. Photography guidance recommends comparable imaging. [1]

Which areas should be documented?

Record the frontal hairline, top views of frontal and midscalp regions, vertex, both temples, and posterior and lateral donor areas. Include the date and time since surgery. Identifiable facial images also require appropriate consent and privacy assessment.

Are photographs enough on their own?

No. They show overall appearance, while unit density, shaft diameter and miniaturisation may require trichoscopy or a phototrichogram. Keep the sampled area consistent and repeat the same technique. [2][3]

Why does hair length matter?

Long hair can cover thinning regions and increase visual coverage. Short hair can reveal pinpoint donor scars or thinning. Consistent length and styling should therefore be the aim.

Are satisfaction and objective results the same?

No. Identical measured density can produce different satisfaction. Expectations, facial harmony of the hairline, shaft thickness and progressive native loss affect the assessment. Satisfaction matters, but does not replace measurable growth.

Can AI-generated images be used?

Illustrative medical images should be clearly labelled illustrative image. A synthetic outcome must not be presented as a real patient’s before-and-after result. Manipulation that increases apparent density can invalidate scientific comparison.

A proposed KSD imaging record

Use consistent camera setup, similar lighting and fixed views, with preoperative and month-three, six and twelve records and defined-area trichoscopy when appropriate. This is a documentation proposal, not an original research finding.

Frequently asked questions

Can a success percentage be calculated from one photograph?

No. Comparable before-and-after images and objective measurements when needed are required.

Are wet-hair photographs more reliable?

Wetness changes coverage. Consistency at both time points is what matters.

Is the month-six photograph a final result?

It is generally an interim result; maturation may continue.
