Donor Reserve and Graft Capacity Calculation

How donor reserve differs from total follicular unit count, and why regional coverage and future sessions matter in graft planning.

Definition and clinical context

Donor reserve is the capacity that can be transplanted while leaving acceptable coverage, rather than the total number of units present in the scalp.[1][2]

Biological and surgical significance

Illustrative calculation: 120 cm² × 70 FU/cm² = 8,400 total units. This is not the number of grafts available for harvesting. Remaining density, regional differences and previous harvesting require separate assessment.[1][2]

Assessment and planning

Hair diameter, curl, hair–skin contrast and hairs per unit change the visual effect of the same graft count.[1][2]

Limits of the evidence

Using the entire reserve in the first session can reduce later revision or crown-treatment options in progressive hair loss. There is no universally safe graft number.[1][2][3]

KSD planning note

Total unit count, the regional harvesting plan and expected residual coverage are recorded separately. The example of 8,400 units must not be used as a surgical target or a personal suitability result.

Topic-specific questions

Do 8,400 total units mean 8,400 harvestable grafts?

No. The example only multiplies area by density; safe harvesting capacity requires a separate clinical assessment.

Does the same graft count give everyone the same coverage?

No. Diameter, curl, contrast, hairs per unit and recipient-area size affect the result.
