Donor Miniaturisation and Long-Term Graft Durability

What donor miniaturisation means and how progressive thinning can affect transplanted hair and the remaining donor coverage.

Definition and clinical context

Miniaturisation is the progressive change from terminal hairs to finer, shorter hairs. Although classic patterned loss mainly affects the front and crown, the occipital and lateral donor areas can also be affected in some people.[1][2]

Biological and surgical significance

Progressive thinning in the source area makes future coverage from transplanted grafts less certain. If the remaining donor hair also becomes finer, FUE marks may become more visible.[1][2]

Assessment and planning

Trichoscopy assesses shaft diameter variation, fine hairs, hairs per unit and regional differences. Telogen effluvium and other alopecias should be considered in the differential diagnosis.[1][2]

Limits of the evidence

There is no single universal miniaturisation percentage that determines safe surgery for every patient. Suspicious findings may require serial observation or postponement of surgery.[1][2][3]

KSD planning note

Planning considers the course of donor thinning as well as the current graft target. Repeated measurements and reassessment of candidacy may be preferable to increasing harvesting from uncertain areas.

Topic-specific questions

Does donor thinning mean transplanted hair will definitely fall out?

No. It increases uncertainty about long-term coverage; its cause and extent require individual assessment.

Can one percentage decide surgical suitability?

No. Distribution, measurement method and clinical progression must be considered together.
